triggered by COVID-19, increased vaccination rates, or a lowered threshold caused by vaccines or previous infection, according to a large case series\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0305417923001286\" target=\"_blank\" rel=\"noopener\">recently published<\/a>\u00a0in the medical journal Burns.<\/p>\n<p>While SJS isn\u2019t caused by fire, it\u2019s usually treated by burn units in hospitals because of its similarities to actual burns.<\/p>\n<p>Researchers with the burn unit at Concord Repatriation General Hospital in Australia saw two to four cases of SJS, or toxic epidermal necrolysis (TEN), per year before COVID-19. In the first six months of 2022 alone, the same burn center observed a sevenfold rise in cases.<\/p>\n<p>Of the 14 reported cases, five patients had COVID-19 a month before developing SJS\/TEN, and three of 14 patients received a COVID-19 vaccine one month prior. Not a single case of SJS\/TEN was reported in an unvaccinated individual.<\/p>\n<p>Researchers said the rarity of the condition and presence of medications known to trigger the disease make the link difficult to prove, but the rapid rise in cases since the beginning of the pandemic and vaccine rollout is \u201calarming.\u201d<\/p>\n<p>SJS\/TEN is a severe hypersensitivity condition\u00a0that causes the skin to develop rashes, blisters, and peels forming painful areas that resemble a severe hot water burn. Mucous membranes, including the eyes, genitalia, and mouth, are often affected or severely damaged, leading to sepsis, pneumonia, infection, or death.<\/p>\n<div id=\"adx_native_ad_111235\">\n<div class=\"adxbs div_111235\">\n<section>\n<div class=\"tn_new_invest\">\n<div class=\"tn_healthy_offer\">\n<div class=\"row\">\n<div class=\"col-xs-7 col-sm-6 col-md-6 col-lg-6\">\n<div class=\"tn_health\">\n<h2>Your Health Matters<\/h2>\n<\/div>\n<\/div>\n<div class=\"col-xs-5 col-sm-6 col-md-6 col-lg-6\">\n<div class=\"tn_for_align\"><span class=\"tn_sponserd\"><a href=\"http:\/\/tapnative.com\/\" target=\"_blank\" rel=\"noopener\">A<\/a><\/span><\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/section>\n<\/div>\n<\/div>\n<p>Although SJS and TEN were once considered separate conditions, they are now considered\u00a0part of the same disease\u2014with SJS representing the less severe end of the disease spectrum and TEN representing the most severe.<\/p>\n<p>Medications, including epilepsy medicines, antibiotics, and anti-inflammatory painkillers, are the chief cause of SJS\/TEN, but certain viruses and vaccines can also cause the condition.\u00a0Because it can be fatal, SJS\/TEN is considered a medical emergency.<\/p>\n<h2>Researchers\u2019 Theories for Sudden Rise<\/h2>\n<p>The researchers proposed three theories for the sudden increase in SJS\/TEN cases.<\/p>\n<h3><strong>Virus-Induced<\/strong><\/h3>\n<p>The first theory is that the SARS-CoV-2 virus induces SJS\/TEN by directly binding to receptors that trigger a T-cell-mediated response. Other viruses known to cause SJS\/TEN include the herpes simplex virus, Epstein-Barr virus, and influenza.<\/p>\n<h3><strong>Vaccine-Induced<\/strong><\/h3>\n<p>A second theory is that COVID-19 vaccines may directly bind to cell receptors that trigger SJS\/TEN and influence the body\u2019s T-cell immune response, initiating SJS\/TEN. This T-cell response peaks at seven and 28 days post-vaccination, consistent with the observed cases.<\/p>\n<p>Of the three cases attributed to vaccination in the study, two patients had received an mRNA vaccine, and one received a viral vector vaccine within a month of developing SJS\/TEN. Researchers identified eight other cases of SJS following COVID-19 vaccination in published literature\u2014four were associated with mRNA vaccines, three with viral vector vaccines such as AstraZeneca and Johnson &amp; Johnson, and one with a whole virus vaccine.<\/p>\n<p>According to the U.S. Vaccine Adverse Event Reporting System (VAERS), <a href=\"https:\/\/medalerts.org\/vaersdb\/findfield.php?TABLE=ON&amp;GROUP1=AGE&amp;EVENTS=ON&amp;SYMPTOMS[]=Stevens-Johnson+syndrome+%2810042033%29&amp;SYMPTOMS[]=Toxic+epidermal+necrolysis+%2810044223%29&amp;VAX[]=COVID19&amp;VAX[]=COVID19-2\" target=\"_blank\" rel=\"noopener\">198 cases<\/a> of SJS\/TEN following COVID-19 vaccination were reported between Dec. 14, 2019, and June 23, 2023. Historically, VAERS has been shown to report fewer than\u00a0<a href=\"https:\/\/digital.ahrq.gov\/sites\/default\/files\/docs\/publication\/r18hs017045-lazarus-final-report-2011.pdf\" target=\"_blank\" rel=\"noopener\">1 percent of actual vaccine adverse events,<\/a>\u00a0which means other cases of SJS\/TEN may have occurred but were unreported.<\/p>\n<h3 id=\"sect0095\" class=\"u-h4 u-margin-l-top u-margin-xs-bottom\"><strong>Threshold Lowering<\/strong><\/h3>\n<p>The third theory proposed by the authors is that developing a COVID-19 infection or receiving a vaccine \u201cprimes\u201d the immune system, lowering the threshold for a drug to trigger SJS\/TEN. Without this \u201cpriming,\u201d the drug would otherwise not cause the condition, as noted in each of the following cases:<\/p>\n<ul>\n<li>A 26-year-old male received two previous doses of a viral vector vaccine and one mRNA vaccine dose. After experiencing vaccine-associated symptoms, he took paracetamol and ibuprofen and developed SJS. He had previously taken both medications with no adverse effects.<\/li>\n<li>A 60-year-old female had COVID-19 six weeks before the onset of SJS\/TEN. During that time, she received allopurinol for gout and experienced a reaction despite that she had taken the drug in the past with no ill effect. The woman had received two doses of an mRNA vaccine.<\/li>\n<li>A 45-year-old male developed a reaction to a seizure prophylaxis drug. He was diagnosed with COVID-19 four weeks before developing SJS\/TEN and was triple vaccinated with an mRNA vaccine.<\/li>\n<li>A 53-year-old female received a viral vector vaccine three weeks before the onset of SJS\/TEN.
